From there, all that you have to do is fill out a short form and include our details (LIFE’s EIN: 95-4402149). You must also be sure to submit proof of your donation. Your company will send the matched donation directly to us once verified. Some companies use matching gift platforms such as Benevity or YourCause to streamline this process.
